Vaccination

You can get your flu shot right in your doctors office. Internal Medicine/Pediatrics is offering the flu vaccine for their established patients. Talk to your provider about getting the vaccine.

Call 813-259-8700. Please fill out a new patient information form and bring it with you to your first appointment. You can either download one from the website or obtain one at our clinic. Please bring all of your medications to each appointment.

Copies of your medical records assist us in understanding you as a patient. If you have copies of your past medical records, please bring these with you. For all records not in your possession, please download a copy of our release of information form, fill it out, and bring it to your first appointment. Medical records can be faxed to us at 813-821-8302.
We accept most insurance, including Medicaid and Medicare, but it's best practice for you to call your insurance and double-check.
Copays vary depending on insurance coverage. Contact your carrier for specific copays.
