Call 813-821-8700. Please fill out a new patient information form and bring it with you to your first appointment. You can either download one from the website or obtain one at our clinic. Please bring all of your medications to each appointment.

Copies of your medical records assist us in understanding you as a patient. If you have copies of your past medical records, please bring these with you. For all records not in your possession, please download a copy of our release of information form, fill it out, and bring it to your first appointment.
