Connect with us wherever you go. You can safely and securely access MyChart from your iPhone or Android.

MyChart is our free online portal offering patients personalized and secure access to portions of your medical records, from the comfort of your own home. Join the new generation of patients taking a more active role in managing their health and wellness.

USF Health strongly encourages all patients to sign up for MyChart, all you need to do is request an activation code.
This code will enable you to log in and create your own user ID and password.

The activation code can be created by:

  • Request a code from your healthcare team at your next appointment, in person or via Telehealth

The 21st Century Cares Act: What this means for you

USF Health patients will now have expanded MyChart-based access to most of their clinical data.  A new federal rule called the 21st Century Cures Act, which is going into effect in 2021 is designed to give patients more control over their personal health information.

Starting on Nov. 2, 2020, USF Health will provide nearly all physician-generated clinical documentation related to your visits with USF Health providers and release them to you via MyChart. Once your physician completes the notes and documentation associated with your USF Health appointment, those details will automatically be sent to you’re your MyChart account.

This more complete documentation will include lab results, imaging results, and pathology results, along with completed notes and orders related to your visits with USF Health providers.

Now offering eCheck-in feature on MyChart, our online patient portal.

Now available through MyChart, eCheck-in allows you to to conveniently:

  • Pay visit copays
  • Update your demographics
  • Verify your medical insurance information
  • Respond to questionnaires
  • Electronically sign consents
  • Limit contact when possible


How do I use eCheck-In?

When you receive your reminder notice about an upcoming appointment, you will be asked to log in to MyChart and select the appropriate upcoming appointment. Click on the UPDATE INFORMATION button to begin eCheck-In process.

Not on MyChart? Go to MyChart and click the “sign up” button.  Once you complete the process, please stop by the front desk to let us know you’ve arrived. eCheck-in online at home or by smartphone and save time when you arrive at our clinic for your appointment.

If you are connecting via Telehealth, also known as a video visit, please complete eCheck-in before your appointment.

USF Health + Tampa General Hospital

Our affiliation with Tampa General Hospital gives our patients access to their health records from both organizations through one MyChart Account. If you currently have a MyChart account with Tampa General hospital, you do not need an account at USF Health.

Your privacy is important to us

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal user names, and passwords. Each person controls their password, and the account cannot be accessed without that password. Unlike conventional email, all MyChart messaging is done while you are securely logged on to our website.

USF Health has added a new feature where you can directly request a copy of your health medical records. To request a copy, please visit USF Health Medical Records.
