Physicians wishing to refer patients to USF Health Maternal Fetal Medicine for an ultrasound or consult:

Please submit a copy of the following along with the MFM Referral Form:

• Demographics
• Copy of Insurance Card
• Medical Records
• Prenatal Labs (including genetic screening) 
• Ultrasound Reports

Please send the MFM Referral and Perinatology Intake Forms to Fax - (813) 821-8391. Allow 72 hours until the appointment is scheduled, patients will receive a call regarding the details of the appointment. The referring office will receive a fax confirmation of the appointment date and time.
