If you are a new patient to our main USF Health Psychiatry Clinic, please review the forms below prior to your visit. These forms will be provided to you at check-in or sent electronically through MyChart.

If you are a new patient to the Memory Disorders Clinic, your new patient forms will be mailed to you before your appointment. Please complete them and bring them to your visit.

If you are a new patient to our main USF Health Psychiatry Clinic, please review the forms below prior to your visit. These forms will be provided to you at check-in or sent electronically through MyChart.

Please note that minors must be accompanied by a biological parent or legal guardian. If the person accompanying the child is a legal guardian, paperwork showing the person’s capacity to consent for care must be presented at the time of the appointment. Similarly, if either biological parent has lost the right to consent for medical care, this legal documentation should be presented at the time of your visit.

Your clinician may request that you complete rating skills periodically to help assess symptom level or progress.

Release of Information

If you want your clinician to speak to someone outside of USF Health about you or your child’s protected health information (PHI) please download the “Release of Information” form located at the bottom of this page.
