If you are a new patient to our main USF Health Psychiatry Clinic, please print both sets of forms below and bring them with you to your visit.

If your child is a new patient, please print and complete both sets of forms below and bring them with you to your first visit.
Please note that minors must be accompanied by a biological parent or legal guardian. If the person accompanying the child is a legal guardian, paperwork showing the person’s capacity to consent for care must be presented at the time of the appointment. Similarly, if either biological parent has lost the right to consent for medical care, this legal documentation should be presented at the time of your visit.
Symptom Rating Scales
Your clinician may request that you complete rating skills periodically to help assess symptom level or progress.
