• List all the questions you have about your health.  
• Let staff know if you have visited other health care providers since the last call, including urgent care and emergency room visits.

• When you want to better understand your individual care plan or health conditions. 
• When you have a question about your care and need an answer. 
• When you see a new provider or start a new medication.
• To alert us you are going to the hospital or are at the hospital.

You have the option to end enrollment in the program at any time. Please call (813) 974-2201 for more information on how to terminate enrollment.
