Chronic Care Management Program

The Chronic Care Management Program (CCM) at USF Health offers support and care to patients diagnosed with chronic condition(s). Our goal is to help patients improve their quality of life by helping them manage chronic condition(s) in their everyday life, including coordinating care amongst their team of providers when necessary.

Our team can help patients:

  • Be more informed about their health care decisions by educating them on the benefits and risks of treatment(s).
  • Review and manage medications to avoid complications.
  • Avoid trips to the emergency room.
  • Collect, share and update health information with the entire team of providers so they can be fully informed of the patient’s health and identify potential issues.
  • Reduce out-of-pocket costs by avoiding duplicate tests and treatments.
  • Get health questions answered.
  • Increase communication and develop a strong relationship between the doctor(s), family, caregivers and themselves.
  • Schedule or reschedule appointments, including preventative care services.
  • Share their medical information with emergency responders.
  • Find community health resources such as fitness classes, transportation services and prescription discounts.

Nurse Assessment Call Checklist 

Before Your Call

  • Have a list of all medications ready, including over-thecounter, herbal and dietary supplements.
  • Inform the CCM nurse of any refills you need.

During Your Call

  • Inform the CCM nurse of any other health care providers you have visited in the last month and the reason for your visit. This includes urgent care or the emergency room.
  • Inform the CCM nurse of any new health problems that may have developed in the last month. Confirm the date of your next CCM nurse assessment call as well as the date of your next office visit with us.

After Your Call

  • Register for MyChart by visiting mychart.health.usf.edu. This is a great way to communicate with your doctor and CCM nurse as well as view your care plan.
  • If you want to designate a caregiver to have access to your CCM medical record, please ask our office for the forms to sign. 

During your initial meeting with us, we will provide you with a dedicated phone number to call between 8:00 a.m. and 5:00 p.m. For assistance after 5:00 p.m., please call 813-974-2201 to reach the after-hours triage nurse.
