Request Your Degree Verification/Certification

Are you a USF College of Nursing alumnus or alumna needing proof of your degree? Our official degree verification process provides certification for employment, licensure, and continuing education.

The step-by-step Degree Verification Instructions are provided below.

Degree Verification Instructions

  • Send your request to con-gradforms@usf.edu. **Do Not Send your form/request to the Dean of the College of Nursing. Forms must be signed by the Dean of the Graduate Program

    REQUEST REQUIREMENTS:

    1. Subject Line: Graduate Verification Request [Insert Type of Request: e.g. Board of Nursing, Employer, Credentialing, etc.]
    2. Full Name including previous names used while in program
    3. U Number
    4. Program Concentration (if applicable)
    5. Attach Form or Request Details in body of email

    Please note:

    • If the request is not from the student (i.e. third party requestor), USF cannot release any student information unless a signed release of information authorization form is completed by the student.
    • Your form will be processed in the order in which it is received, which typically takes 10 business days.  Complicated forms may require additional processing time and may exceed 10 business days.
    • For questions regarding initial request or the status of your form please email con-gradforms@usf.edu
    • STEP ONE: Please obtain the following forms:
      • Verification form (with graduate’s information completed)
    • STEP TWO: Request Official Transcripts indicating that you have a Supplemental Form:
      • Upload the supplemental form to your transcript request
    • STEP THREE: The Registrars Office will notify the College of Nursing to complete the supplemental form and will be sent back to Registrar's Office to include with your official transcript.
      • Requests are processed in the order in which it was received, which typically takes 3-4 weeks, and will be provided to the Registrars Office to include with your official transcript.
