Welcome to the USF Health Medical Health Administration (MHA) department. The MHA departments purpose is to identify employees, physicians, medical students, health students, and allied health professionals who could be at risk for preventable communicable diseases. Our goal is to minimize the transmission of communicable diseases to and from employees, students, and patients detailed on our Exposures and Resources webpages.

It is required that all health professions students, residents, fellows, and clinical faculty /staff provide proof of influenza immunization each academic year to the USF Health Medical Health Administration office. If you are declining the annual flu immunization you must sign and submit the declination form (completed in Qualtrics), which will auto send to MHADocuments@usf.edu.
