• Preterm birth less than 30 weeks gestation

• Hospice status

• Transplant history

• Neurologic conditions: significant seizures, hypoxic ischemic encephalopathy, traumatic brain injury,

  developmental delay, cerebral palsy, spina bifida, neurodegenerative disorders or other neurological conditions

• Respiratory conditions: oxygen dependence, chronic lung disease, cystic fibrosis or other respiratory diagnosis

• Cardiac conditions: cardiac medication dependent, congenital heart disease or other cardiac diagnosis

• Gastrointestinal issues: short bowel syndrome, significant dysphagia, failure to thrive, TPN dependence, tube feeds, ostomy status or other GI diagnosis

• ENT conditions: cleft palate, cystic hygromas or other ENT diagnosis

• Hematology/oncology conditions: Sickle cell disease, significant hemoglobinopathies, malignancies or other hematology/oncology diagnosis

• Endocrine conditions: panhypopituitarism or other endocrine diagnosis

• Renal conditions: chronic kidney disease or other renal diagnosis

• Genetic conditions: metabolic disorders (require follow up with 2 or more specialists), genetic abnormality (require long term follow up) including but not limited to: trisomy 13/18/21, Prader Willi syndrome, William’s syndrome and Turner’s syndrome.

• Musculoskeletal conditions: osteogenesis imperfecta, significant skeletal dysplasia’s, significant anatomical abnormalities (require adaptive technology or long-term interventions) or other musculoskeletal diagnosis

• Dermatology conditions: epidermolysis bullosa or other dermatology diagnosis

• Adaptive technology: neurological shunt, feeding tubes, tracheostomy, ventilator, wheelchair, central line, oxygen

Our Pediatric Complex Care patient navigator, works closely with inpatient care teams, patients, and families to help ensure a smooth and coordinated hospital experience.

She supports children with complex medical needs and their families by helping navigate the many moving parts of a hospital stay, facilitating communication across inpatient teams, and ensuring families stay informed and supported throughout admission. As a consistent point of contact during any hospitalization, your patient navigator helps coordinate the flow of information, provides ongoing support, and prepares families for a safe and well-coordinated transition home.

With a strong focus on clarity, continuity, and family-centered support, our goal is to reduce stress during hospitalization and ensure families feel informed, supported, and confident as their child transitions to the next phase of care.
