Perinatal Mental Health

Mental health conditions are among the most common complications of pregnancy and the postpartum period, affecting approximately 1 in 5 women. While these conditions are often treatable with timely support and care, they remain a significant maternal health and patient safety concern. Hospitals and community partners play an important role in supporting maternal health through screening, early recognition, and connections to appropriate care and resources. FPQC recognizes maternal mental health as an essential component of respectful maternity care and prioritizes it across our quality improvement efforts.

This webpage provides practical resources to help hospital teams strengthen mental health screenings and referral processes, connect patients with support, and integrate with community-based services across Florida.

Screen and Refer Patients for Care

    • Edinburgh Postnatal Depression Scale (EPDS): Ten short statements to help health professionals in detecting mothers suffering from PPD. Takes approximately five minutes to administer.

    • Patient Health Questionnaire (PHQ-2): A brief, two question screening tool used to identify patients who may be experiencing depression.

    • Depression Screening Tool (WCF): Eight question screening tool adapted from the PHQ9 by Women’s Care Florida.

    • PRAPARE Screening Tool: A national standardized patient risk screening for social determinants available in 25 languages.

    • Substance Use

      • Screening, Brief Intervention, Referral to Treatment (SBIRT): This is a comprehensive, integrated, public health approach to the delivery of early intervention and treatment services for persons with substance use disorders (SUD), as well as those who are at risk of developing these disorders. A validated SUD screening tool should be integrated into the first step of this process.

      • 5P's Screening Tool and Follow-up Questions: Designed specifically for pregnant women, this tool asks about substance use by women’s parents, peers, partner, during pregnancy, and in her past.

      • CRAFFT:  This provider guide for the CRAFFT screening tool offers information on how to use the screen. The CRAFFT is specifically for use with patients under 21 years of age.

    • PSI Perinatal Providers Psychiatric Consult Line:  Free consultation line available for medical professionals who have questions about mental health care related to pregnant and postpartum patients and preconception planning.     • Florida BH IMPACT Program: Clinical psychiatric consultation for pregnant and postpartum women with substance use or mental health disorders, patient resources and referral available.

    • ACOG Perinatal Mental Health Guidance for OBGYNs: Evidence-based clinical guidance from ACOG on screening, assessment, treatment, and follow-up for perinatal mental health conditions in obstetric practice.

    • ACOG Perinatal Mental Health Tool Kit: A practical toolkit with screening recommendations, treatment guidance, referral strategies, and patient education resources to support the integration of perinatal mental health care into clinical practice.

    • AIM Perinatal Mental Health Conditions Bundle: A quality improvement resource that provides hospitals with evidence-based practices to improve the identification, treatment, and management of perinatal mental health conditions through standardized protocols and multidisciplinary care.

    • Postpartum Support International (PSI) Helpline: Connects pregnant and postpartum individuals with information, local resources and support. Call or text 1-800-944-4773 (English) or 971-203-7773 (Spanish), available 8:00 a.m. - 11:00 p.m. ET. This is not a crisis hotline.

    • National Maternal Mental Health Hotline: Free, confidential support for pregnant and postpartum individuals and their families. Call or text 1-833-TLC-MAMA (1-833-852-6262), available 24/7, with interpreters in 60+ languages.

    • If someone is experiencing a mental health emergency or is at immediate risk of harm, call 911 or contact the 988 Suicide & Crisis Lifeline by calling or texting 988.

    • PSI Online Support Groups Virtual support groups on a variety of topics.

    • Statewide Resource Guide:  Provides information on more than 100 active organizations, support groups, non-profits, and medical and therapeutic centers dedicated to providing advocacy, information, health care, support, recreational activities, etc. for children with special healthcare needs and their families. Guides are updated annually.

    • Preeclampsia Foundation Birth Trauma Resources: Educational resources designed to help patients and families understand, process, and recover from traumatic birth experiences.

    • NICU-Specific

      • March of Dimes NICU Family Support: Provides education, emotional support, and family-centered resources for parents during a baby’s NICU stay and transition home. The program offers training and quality improvement support for NICU staff. 

      • NICU Parent Network: A national network of parent-led organizations that connects NICU families with peer support, education, and community resources.

      • Graham’s Foundation NICU-Family Resources: Offers support for families of premature infants through educational resources, trained peer mentors, free care packages, online communities, and tools for navigating the NICU and life after discharge.

      • NICU Alumni: Provides education, support groups, and practical resources for families adjusting to life after the NICU, including guidance on development, follow-up care, and connections to supportive services.

    • “Say This/Not That”: A poster demonstrating non-stigmatizing language to use when discussing substance use disorders.

    • Beyond Labels: This March of Dimes toolkit is designed to raise awareness about stigma and provides some sample language, stories about stigma from women and healthcare providers, and a very nice list of resources on bias and stigma.

    • Blooming Littles for Parents and Clinicians: Offers evidence-based resources, training, and education to help NICU families and clinicians create supportive, neuro-nurturing care experiences.

    • Baby Steps to Home: Free online resource from NANN available to all hospitals across the country. Topics in the module have sections with content for both nurses and parents.

    • Hand to Hold App:  A free smartphone app endorsed by the NPA that offers support groups, counseling, educational content, and other resources to NICU families and providers.
