Thank you for your interest in the Clinical Neurophysiology Fellowship. Due to a limited number of spots, we highly encourage prospective fellows to apply as early as possible. Consideration for an interview will be given upon submission of all required documents.

The required documents for the Clinical Neurophysiology Fellowship include:

• Completed CNPF application

• Curriculum Vitae

• Personal Statement

• Copies of USMLEs (for DO's -copies of COMLEX)

• Copy of Medical School Diploma

• Copy of Medical School Transcripts 

• Copy of Educational Commission for Foreign Medical Graduates (ECFMG) Certificate (International medical graduates only)**

• If currently in a Residency Program, letter from your Program Director. If you have completed a Neurology Residency, then submit a copy of your Certificate of Completion.

• Three (3) letters of recommendation, dated within the last 6 months. Your Program Director letter will count as one of these.

• Copy of valid Passport
