Authorized USF Health staff should send completed Payment Request Forms (PRFs) as attachments to health-finaid@usf.edu.

To assist us with processing PRF’s accurately and timely, please ensure:

• The email is sent from a @usf.edu account
• The USF Health Financial Aid Payment Request Form (PRF) is fully completed
• The sender is an authorized USF staff member

Helpful Reminders:

• Submit only one fund code per form/document to ensure accurate and timely processing. Multiple fund codes should be submitted as separate forms.
• Please send forms directly to health-finaid@usf.edu rather than individual team members to avoid delays.
• Emails with PRFs are reviewed in the order they are received. Please allow time for processing, which may take up to 15 business days during peak periods.

Processing Timeline for 2025–2026 PRF Submissions

To support timely disbursement and to minimize the impact of possible over award situations for students, please submit all 2025–2026 award requests by the following deadlines:

• Fall 2025: Submit by December 11, 2025
• Spring 2026: Submit by May 7, 2026
• Summer 2026: Submit by August 7, 2026

These deadlines support timely disbursement and reduce processing delays during peak periods. We encourage submissions prior to the beginning of a semester.
