*Value-based care is a healthcare model that rewards providers for delivering high-quality, patient-centered and cost-efficient care – linking payment to outcomes rather than the volume of services delivered.
Examples of value-based care agreements include: Accountable Care Organizations (ACOs; CMS, ACO, REACH, MSSP), shared savings/shared risk contracts, bundled payment models, capitated or population-based payment arrangements (MA, commercial risk), and pay-for-performance (P4P) agreements.
