Preparing for Your First Visit
When you come to your outpatient appointment, please bring the following so we can provide you the best quality care:
- Your insurance card.
- Physician referral forms if required by insurance.
- A list of current medications both prescription and over-the-counter
- CD-ROMs or x-ray films of any imaging studies performed, such as CT or MRI scans (we will review the images themselves, not just written reports describing the films).
