USF Health Scholarship Forms
The USF Health Office of Financial Aid partners with authorized USF Health faculty and staff to facilitate timely and accurate scholarship processing. Completed Payment Request Forms (PRFs) should be submitted to health-finaid@usf.edu in accordance with the guidance outlined below.

To ensure Payment Request Forms (PRFs) are processed accurately and efficiently, please confirm that:
The email is sent from a @usf.edu account.
The USF Health Financial Aid Payment Request Form (PRF) is fully completed.
The request is submitted by an authorized USF Health faculty or staff member.

The following reminders are intended to support efficient processing and minimize delays for both departments and students:
Submit only one fund code per form. Requests involving multiple fund codes should be submitted using separate forms.
Send forms directly to health-finaid@usf.edu rather than individual team members to help avoid processing delays.
PRFs are reviewed in the order they are received.
During peak processing periods, please allow up to 15 business days for review and processing.
Whenever possible, submit scholarship requests before the beginning of the semester to support timely disbursement.

To support timely disbursement and minimize the potential impact of overaward situations, scholarship requests should be submitted by the deadlines listed below.
Fall 2026: Submit by November 20, 2026
Spring 2027: Submit by April 23, 2027
Summer 2027: Submit by June 25, 2027
These timelines help support accurate awarding and reduce processing delays during peak periods. Whenever possible, departments are encouraged to submit scholarship requests prior to the beginning of the semester to facilitate timely disbursement.

Students with questions regarding scholarship eligibility, financial aid awards, or financial aid requirements should contact the USF Health Office of Financial Aid using the student contact information available on the Financial Aid website.
To help ensure timely responses and appropriate routing, please do not send scholarship processing questions through Microsoft Teams.
