Employment Verification

Requests for basic employment verification or personnel records for USF Health Faculty for   (Dates of Employment, Job Title, Annual Salary) can be submitted via email to the Office of Faculty Affairs (OFAA) at OFAA-records@usf.edu.

The OFAA will monitor & process USF Health Faculty Records requests sent to this email. Please allow up to seven business days for these requests to be processed.

We will provide records for faculty associated with:

USF Health Morsani College of Medicine

USF Health College of Nursing

USF Health College of Public Health

USF Health Taneja College of Pharmacy
