New Patients

We know you have many choices in your health care providers and we are honored to have your trust. The Byrd Alzheimer’s Center is part of the USF Health Neuroscience Institute which enables us to leverage the expertise of our colleagues for your benefit. Our expert team includes physicians, nurse practitioners, nurses, clinical staff and educators all dedicated to helping both patients and their families. Your symptoms and experiences are unique and we believe the care that we provide should be equally personalized.

Making an Appointment

Our care providers accept patients who are demonstrating signs of memory loss, dementia or early stage Alzheimer's disease.  All new patients must first complete a New Patient Appointment Request Form.  Print and complete the form and return it to our office by mail or fax, or by dropping it off to our clinic receptionist. Our address and fax are on the form.

Once we receive your completed form, it will be reviewed and your insurance will be verified.  One of our client service representatives will then contact you to set up an appointment with a provider who can best meet your needs.

Referrals

Some insurance carriers require a referral in order to receive reimbursement or coverage. Our staff will review your New Patient Appointment Request Form and notify you if a referral is necessary.

When Coming to an Appointment

When coming for an appointment, patients should be accompanied by a family member, loved-one or primary caregiver. Please arrive 30 minutes prior to your appointment to allow time to check in and complete the registration process. Please bring your:

  • Insurance cards
  • Relevant medical records
  • All medications in their original bottles
  • Any recent imaging (on disc)

PLEASE NOTE: No more than two people can accompany the patient to their appointment.

Parking

  • Complementary valet parking is provided for all patients and their families coming to the Institute for an appointment. Please drive to the font of the building to access the valet.
  • Handicap Spaces in the parking lot are for staff only. Patients and families  with a handicap tag should use the valet parking  so you do not have to walk up the sidewalk.

  • Other Parking Options: if you do not want to use the valet parking, you will need to park in the Laurel Street Garage located behind our building. You can then enter the Byrd Institute lobby from the back of the building. You will have to go up the stairs and enter through the double doors.

  • Please Note: The parking spaces marked "E parking" and all the spaces in Lot  43 (the lot adjacent to the Byrd Institute) are reserved for faculty, staff and students. Patients parking in  E spaces or Lot 43 are subject to a ticket.

Insurance

USF Health Byrd Alzheimer's Center accepts most major insurance plans. While we do not require a referral, we encourage our patients and caregivers to verify with their insurance carrier that the physician they will be seeing is included on their plan. Pre-authorization is required by some insurance plans. Please bring your insurance cards with you to your appointment.

Clinic Hours

Our clinic is open Monday- Friday from 8:00 a.m. to 5:00 p.m.
