The following are some resources available as you see fit based on the program you implement.

Pre-Program and Post Program Survey

The FPRC team has created a survey that can be administered to collect data before the start of any program activities, and then again after the completion of all program activities. The survey was created to assess the employee’s knowledge, awareness, and perception of colorectal cancer screening. You can utilize sites like Survey Monkey, Google Forms, Qualtrics, or any other survey platform you have access to at your worksite.

Event Based Evaluation Items

  • Event Evaluation Form (Two-question feedback sheet to elaborate how well the event was received by the employees.)
  • Event Report (For employers to monitor and keep track of material that may have been distributed and logistics of event.)
