According to Centers for Disease Control and Prevention (CDC), “Social determinants of health (SDOH) are conditions in the places where people live, learn, work, and play that affect a wide range of health and quality-of life-risks and outcomes.”
FPQC is working with stakeholders (patients, families, clinicians, and community organizations) to form an advisory group and develop a quality improvement initiative to assist hospitals in addressing a myriad of organizational, social and environmental factors that influence maternal health behaviors and outcomes. The hospital kickoff is planned for March 2023.
