Privacy incidents or breaches must be reported using our incident reporting system, PrivacyPro Solutions If you believe a breach or privacy incident has occurred, please complete the following steps:

1. Please contact the Purchasing Manager so a courier can be ordered to pick up the original disclosed PHI (if in paper form) and returned to USF.
