Clinical System Access Request

Description

USF Health supports and hosts systems which support the healthcare mission and to which Health account holders can be granted access. Permissions to these resources are provided centrally by Health IS with the proper approvals.

How to Request this Service

Epic/GE/Allscripts/GE Centricity Business: Follow the request instructions. Keep in mind that there is a form to complete and required training (online and in-class for some access levels).

Periop: email Adele Emery for access request: aemery@health.usf.edu 

For all other systems: submit an account modification request under the system owner's department, and then select the "other" category. Include the following information: the system you request access to, the level of access needed, and reason for access.  For more information on how to fill out an account modification request, see the IS Documentation Article

Availability

Requests can be submitted 24x7 through the account modification request screen.
