Business System Access Request
USF Health supports and hosts many business systems to which Health account holders can be granted access. Access to these resources are provided centrally by Health IS with the proper approvals.
How to Request This Service
CODA: Fill out access form and email completed form to firstname.lastname@example.org. Note: only a current authorizer for the department indicated can countersign the form.
Cyborg: Departmental supervisor must email email@example.com with access details
Cactus: contact Fran Isbell in Managed Care (firstname.lastname@example.org)
GEMS/FAST/other USF-wide systems: See IT's Service Catalog
All other systems: submit an account modification request under the system owner's department, and then select the "other" category. Include the following information:
- the system you request access to
- the level of access needed
- and reason for access

Requests can be submitted 24x7 through the account modification request screen.
