Collection Policy

Please contact the USF Health Endoscopy and Surgery Center prior to your scheduled procedure for pre-admission instructions and final arrangements. PLEASE NOTE: ALL FINANCIAL ARRANGEMENTS MUST BE MADE PRIOR TO SURGERY. Financial specialists for the Surgery Center can be reached at: (813) 396-2386 and (813) 396-2389. Your insurance benefits will be verified prior to your surgery date and you will be notified of the payment due based on expenses not covered by your insurance, including deductibles, co-payments, etc. Payment for out-of-pocket expenses will be expected prior to surgery.

Uninsured and cash paying patients will be required to pay for services prior to the procedure date. We do accept most credit cards.The Center will bill your insurance company as a courtesy; however, any balance due is your responsibility and will be requested from you if we have not received payment from your insurance company after 60 days.

Your Surgery Center Fee Includes:

  • Medical and surgical care
  • Cost of routine equipment and supplies
  • Cost of drugs and anesthesia administered
  • Operating room time
  • Recovery room services

Anesthesia, pathology, surgeon, and physician consultation fees are not included in the Center’s fee, and may or may not participate with the same health insurers or health maintenance organizations as the surgery center.

Prior to your procedure, you may request from the surgery center and other health care providers, a more personalized estimate of charges and other information. The estimate is based upon your current insurance benefits. Patients should contact each health care practitioner who will provide services in this surgery center to determine the health insurers and health maintenance organizations (HMOs) with which the health care practitioner participates.

Following your procedure, upon request, you may request an itemized bill of charges.

Financial Assistance Policy

Patients who are not eligible to receive services paid for by insurance or other third party payment sources may be eligible to receive an uninsured discount from our facility. This discount does not apply for cosmetic surgery services as defined by our facility. The discount is a set percentage off of charges and is subject to change. Please contact our Financial Specialists for more information. 

Charity Care:

The center maintains a charity discount policy which provides financial relief to patients who receive medically necessary care and who do not qualify for state or Federal assistance and are unable to pay the estimated or remaining financial responsibility in part or in full. A patient must meet the policy’s household income qualifications which are based on national poverty guidelines established by the U.S. Department of Health and Human Services which are updated annually. Submission of supporting documentation is required to validate a patient’s qualifying status.
