• History Pre-Registration form through One Medical Passport: All patients must complete information within our One Medical Passport patient portal. You will need to create a login and provide, among other things, a listing of your current medications and medical history information that is pertinent to a pre-operative / pre-procedure assessment. Once you have completed it, keep your login and password – if you ever have a procedure or surgery again at the Morsani Center, you will only need to update your profile.
