Program Registration
To register, you need to follow the following steps:

Click HERE to create a USF Health ID (which will be used to create your mentorship profile).

Check your email to confirm account.

Create a mentorship profile. You will be asked to answer a series of questions which will be used to match you with your mentorship partner.
The FPHTC Online Mentor Program Portal is also where you will find a list of your potential matches, track all mentorship activities and complete the required mid-program and final program evaluations.
Additional program instructions will be provided via email once you have enrolled and completed your profile.
