The Florida Perinatal Quality Collaborative will include both charter and general members. Charter members include statewide organizations that represent the broad spectrum of individuals, professions and organizations necessary to improve maternal and infant health care quality. Charter members constitute the FPQC Steering Committee. The Steering Committee can add new charter members as necessary.

General Membership Categories

Fees for general membership in the FPQC are annual payments from the start date of payment received.

General memberships in the FPQC are available in the following categories:
Individuals
Any individual is eligible for membership. Individual members will receive regular communications from the FPQC such as newsletters, meeting notices, opportunities for continuing education and updates on quality improvement initiatives. The enrollment contribution for individual membership is $25.
Professionals
Any perinatal health-related professional (medical, nursing, administrative, social work, etc.) is eligible for membership. Membership entitles the professional to advance notice of FPQC educational programs as well as routine communications regarding FPQC leadership opportunities, meeting notices, continuing education activities and updates on quality improvement initiatives. The enrollment contribution for professional membership is $25.
Hospitals/Corporate

Any hospital or corporate organization concerned with the quality of perinatal care in Florida is eligible for membership. Hospitals/Corporate membership will entitle all interested employees at that organization to the benefits available at the Hospitals/Corporate level. This membership will include advance notice of FPQC education programs as well as routine communications regarding leadership opportunities, meeting notices, continuing education activities and updates on quality improvement initiatives. The enrollment contribution for Hospitals/Corporate membership is $500.

General Membership Categories Benefits:
After the FPQC’s Governance Meeting in late September 2012, the FPQC Leadership Staff and Team continue to analyze and develop the value added differences and appropriate price points for each type of general membership category based on attendees’ feedback. The above version is still in draft form. Final decisions will be made by April 2013.

Existing General Membership Fees:
Those FPQC contacts who have joined already or are planning to join the FPQC before April 2013 will not have a start date for the annual fee until April 2013.

Hospitals/Corporate Note:
Hospitals/Corporate Membership in the Florida Perinatal Quality Collaborative (FPQC) at the USF Chiles Center within the USF College of Public Health is not related in any way to the provision of medical services but provides charitable support for health care quality improvements through education and collaboration in learning activities. In particular, corporate membership and support of the FPQC does not relate in any way to any medical or other health-related services provided by any party, any affiliation or other services agreement between any parties or the referral of patients (if any) between a corporate member and the USF Morsani College of Medicine. By becoming a corporate member, you will receive advance notice of educational programs for your health care professional staff as well as routine communications regarding FPQC leadership opportunities, continuing professional education activities and updates on perinatal quality improvement initiatives. Donations are used solely to provide educational programs and opportunities for health professional staff including training scholarships and are not used in support of any USF Health clinical facilities, salary or benefits for any USF employed or contracted medical professionals. The USF Foundation is a not for profit, tax exempt, direct-support organization of the University of South Florida.

Subcommittees Note:
If possible, the Subcommittees will consist of a variety of health care practitioners as well as advocates, policymakers and payers in order to create a culture of inter-professional expertise and collaboration. Please include your full name, email address and affiliation if you choose to join a Subcommittee. The FPQC will follow up by email to gather further information on your profession and specialty (if applicable).
