New non-degree students and those who have not attended in the past 12 months are required to submit the USF non-degree student application form
online. First time registrants MUST fill out and sign the Mandatory Immunization Health History Form. Off-campus students taking only online classes are temporarily exempt from the immunization requirement. All non-degree seeking students must complete a Non-Degree Student Registration Worksheet for non-core courses (courses that do not require a permit are PHC 6000, PHC 6050, PHC 6102, PHC 6357, and PHC 6410). The Registration Worksheet will allow you to obtain a permit for courses.
Enrolled in an online course?
All online courses are supported by COPH Office of Educational Technology and Assessment (ETA). The ETA office in conjunction with course instructors, prepares Course-at-a-glance documents for all online courses. The CAGs assist students in learning more about the course, its requirements, course expectations, types of assessments and important testing dates for a given semester. To access these documents, go to http://health.usf.edu/publichealth/eta/courses.html and click on the course of interest.
Once registered, students in all online public health courses are expected to meet certain requirements to successfully participate in their courses. Failure to meet these requirements may cause problems accessing the course materials. It is the student's responsibility to ensure all requirements are met prior to the start of the semester. For a list of technology requirements, how to access your online course website, and order course materials, visit: http://health.usf.edu/publichealth/eta/students_tech_requirements.html
If registering for a non-core course, instructor signature required on worksheet. (Students accepted in a certificate program do not need instructor approval). An e-mail from the course instructor giving permission to take the requested classes can be attached to the worksheet in place of a signature. If you are interested in a graduate certificate program you should go to the Graduate Certificate website.
Go to OASIS to verify if permits have been issued (usually within 24 business hours). There will be no e-mail confirmation sent that permits have been issued.
