Sponsored Institute (CDC/USPHS)
Registration procedures for Sponsored Institute students differ from that of traditional students. All new and continuing students should follow the directions in the Registration Packet. Remember to send (mail or fax) the completed Off-Campus Registration Form, Residency Affidavit, and Student Data form in this packet along with proof of your affiliation with a Sponsored Institute participant to the Public Health Practice Office before the semester deadline:

- Once you send in all the completed forms and paid for your courses online, your registration will be completed by the Public Health Practice office.
- Forms to be completed include:
- Off-Campus Standard Calendar Credit Course Registration Form
- Student Residency Affidavit
- Student Data Form
If this is your first time to register as a non-degree student, or if you have not attended in more than a year, instructions are included in the Sponsored Institute Registration Packet. You must complete:
- online USF Graduate School Non-Degree Status Application: https://www.grad.usf.edu/fastapp.html
- Mandatory Medical History Form: http://www.grad.usf.edu/ImmForm.pdf . Off-campus students are temporarily exempt from the immunization requirement. However, first time registrants MUST still fill out and sign the left side of the Mandatory Medical History. Off-campus students who take an on-campus course must meet USF's immunization policy and complete the entire immunization form. Mail or Fax to:

To pay for a Sponsored Institute course, please visit: http://shop.usf.edu. Click on the College of Public Health Store Link and then click on the link for the upcoming semester. Select the course(s) you have chosen to process your payment. MasterCard and VISA are accepted.

Find a list of current online courses at http://health.usf.edu/publichealth/eta/courses.htm. To order textbooks & supplemental readings, click on the desired course and go to the Course-at-a-Glance document for specific details and links to order forms.
