017  Decontamination Fee

A fee that is associated with special decontamination requirements. This fee will be assessed each time decontamination is required. For example, decontamination fee will be assessed prior to and after drug preparation if the work area must be decontaminated per protocol.

018  Therapeutic Dosing/Monitoring Fee

A fee that may be associated with therapeutic dosing, monitoring, interpretation of levels, dosage adjustment due to organ impairment, etc. Time unit is billed in 1-hour increments.

019  Sponsor/Monitor Visits Fee

Imposed for all Sponsor/Monitor visits that occur including site initiation, close-out, and drug accountability visits, sponsor audits, etc. Visits are billed in 1-hour increments with 1-hr minimum billed for each visit.

020  Storage Fee

A one-time fee that will be billed at study close-out, to cover expenses of document storage after study closure. This service is optional. If the Principal investigator requests that the pharmacy maintain records for a said time after study close the outlined fee multiplied by said years will be billed to the study.

021  Miscellaneous destruction fee

A fee developed for the destruction of study drug that may be utilized when investigators have no means of drug destruction.

 022 Pharmacy Close-Out Fee   A fee that is billed at the study close-out to cover all expenses including: product destruction or product return, monitoring fee for close-out visit, document storage after close-out indefinitely and any future requests to supply data from the stored documents  $250-500
