Key or Card Access Request

All USF HEALTH key and card access requests are processed and granted through the Office of Operations and Facilities Management (OFM).

Key Requests

New key requests are made by completing the USF Health Key Request form and submitting it to OFM (attention: Karen May, MDC Box 23). Forms may also be faxed to Karen at 813-974-3846 or e-mailed to kmay@health.usf.edu.  Once the form is submitted and approved, it should take approximately 2-3 days to turnaround your  request.

Card Access Requests

Card Access is requested by an authorized departmental representative via email to kmay@health.usf.edu. These requests are reviewed by OFM to ensure access is authorized. Once authorization is verified and the appropriate data is entered into the system, card activation will normally take approximately 24 hours.
