Prepare for Your Visit

Health Provider Visits: What Should I bring?

  • Write down questions and bring them with you

  • Write down or bring all medications and supplements so the provider will know what you are taking. (This includes vitamins and over the counter medications!)

  • bring your meter(s) and your logbook! If you test and use the memory in your meter, write down results: time tested and in general foods and activities for at least the week prior, and preferably 2 weeks prior to the visit. If you download your meter results on your computer, bring a print out with you and make some notes about diet and exercise to help explain highs and lows.

  • If you’re on the pump, it’s helpful if you write down your carbohydrate ratios, your “correction factor” (target blood glucose and insulin sensitivity) and your basal rates. Although we can find this information on the pump, this expedites your visit.

  • If you’ve been on “diabetes vacation” recently and haven’t been testing your blood sugar, please do some testing in the 2 weeks before your appointment so that your doctor will be able to review your more recent numbers.

  • Please think about when you had your last dilated eye exam so that you can discuss when a follow up exam is needed.

  • bring a recent food history or food diary with you to discuss meal patterns wit h your provider or diabetes educator.
