Application For USF Sleep Fellowship Program
Sleep Medicine and completely separate from the 3-year Pulmonary and Critical Care Fellowship Program. We adhere to a multidisciplinary approach to Sleep Medicine with rotations in pulmonary medicine, ENT surgery, pediatrics, neurology and long-term seizure monitoring, radiology, and dentistry. We strive to provide a balanced training program with emphasis on patient care, education, and research. Applicants must have completed an ACGME approved training program in one of the following areas by the time his/her fellowship is scheduled to begin: Pulmonary Medicine, Internal Medicine, Psychiatry, Neurology, Pediatrics, Otolaryngology, or Family Medicine. We will begin accepting applications for 2010/2011 beginning 3/1/09 until 6/30/09.
Currently, we do not have a formal application but are asking any interested applicant to submit the following documents:

Please note that while the Fellowship is through the University of South Florida, Dr. Anderson, the Fellowship Director, has an office at the VA Hospital and so receives his mail at that address.
NOTE: While we do consider international medical graduates for the program, we do not accept applicants with visas or work permits of any type. It would be necessary for the applicant to be a United States Citizen for his/her application to be accepted and considered.
