Fellowship Application

Thank you for your interest in our Allergy/Immunology Fellowship Program. We are participating in the ERAS match for 2013 and you may go to the ERAS website to complete an application form:  https://www.aamc.org/services/eras/ 

The following supporting documents should be sent to ERAS:

1. Letters of recommendation from:
   • Dean's letter from the medical school you attended
   • Program Director or Co-director of your current or past training program
   • Three other mentors from your current or past training program
2. Curriculum Vitae
3. Medical school transcripts
4. USMLE/COMLEX scores

Supporting documents that do not finish processing through ERAS before the program deadline may be sent or emailed directly to the program coordinator listed below. 

In addition to the completed ERAS on-line application, we also ask that you complete a USF training program-specific application which can be downloaded below.  This application is only for the Pediatric Allergy & Immunology program:

The closing date for 2013 applications and supporting documents is August 1, 2012.
