Departmental Application

We ask that in addition to completing our application process through ERAS that you also mail the following to us:

• Three letters of recommendation addressed to Dr. Mitchel Hoffman
• Letter of good standing from Program Director
• Medical School Diploma 
• Current curriculum vitae
• Personal statement
• Official transcript from all medical schools attended
• USMLE transcript including all attempts
• Recent photograph

Applicants may choose to send the following to supplement their application:

• Transcript of all college education
• Transcript of CREOG scores
• Additional letters of recommendation

Additional documents for foreign medical school applicants – Note: Documents in a language other than English must be submitted with a certified translation.

• Transcript of all college education
• ECFMG certificate

Additional documentation may be required before a candidate can be appointed.
