Incoming 2013-2014 Pharmacy Students

Incoming Pharmacy students for the AcademicYear 2013-2014, prior to beginning training at the University of South Florida College of Medicine and its affiliated institutions, you must complete and return the Communicable Disease Prevention Certification and Physical Examination Verification Forms along with supportive documentation by July 26, 2013.  Patient contact will not be permitted until the form and supportive documentation are complete.  You are urged to obtain documentation from your medical school or current residency program.  All documentation must be in English.

The University of South Florida College of Medicine is unable to provide the vaccines and laboratory titers required for starting your program.  These immunizations and/or laboratory tests must be completed prior to beginning your program.  If you have any questions regarding the Communicable Disease Certification Process or you are not able to receive certain immunizations (ie. they are contraindicated), please contact our office directly at (813) 974-3163 to discuss your situation.  All required vaccines are readily available through your local Health Department.

Current Pharmacy Students

Immunization/Vaccination records on file in the Medical Health Administration Office can be obtained in person or by completing and returning a Medical Record Information Release Form to the Medical Health Administration Office.  Please allow 3 to 5 business days for processing.

PPD/TST (Tuberculin Skin Test) and Influenza vaccinations can be scheduled by calling (813) 974-3163 during office hours.
