Requesting Replacement Certificates
Replacement certifcates for USF Morsani College of Medicine can be ordered through the GME office.
Complete the Certificate Request Form and send to:
USF Health Payment Center
P.O. Box 864300
Orlando, FL 32886-4300
Include a check (made payable to "USF GME") for $25 and indicate "certificate" in the comment section of the check.
Please expect at least a 4 week period between verification and mailing of the certificate.
