     These are our most current policies.  A new one is presented to our affiliated centers each year.

ü       Self-Assessment of Clinical Education Potential form

ü       USF Affiliation Agreement: If you choose to use this agreement, please type in the information required in the first paragraph and on the signature page.  Send two copies with original signatures and a copy of your liability policy to:

You may submit a copy of your company's affiliation agreement to us.  We will review the agreement, obtain the appropriate signatures here, and return the agreement to you for signatures with a copy of our liability policy.  You may submit your affiliation agreement by  mail using the address above or electronically to jpitts@health.usf.edu.

         

Also, you must submit the APTA Clinical Site Information Form (CSIF). This form is an accreditation requirement.  It also provides faculty and students with information about your center.  PLEASE SUBMIT THIS ELECTRONICALLY ONLY (on a disk or CD by mail, or send via e-mail as an attachment to jpitts@health.usf.edu).   You can find it in the "OTHER FORMS" section.
