XX Task Description Due Date
  Physical Exam Students must undergo a physical examination prior to starting school. Complete the attached Physical Exam Verification Form, take it to your health care provider for completion of their section and return it to the Student Affairs Office as soon as possible, but no later than July 18, 2008. 07/18/2008
  POM Name Tag The Office of Educational Affairs will make name tags for each new student which you will be required to wear daily during the first three to four weeks of school. The name tag will contain your official first name as reflected in your BANNER registration. If you prefer to be known by an alternate name (i.e. middle name, nickname) and wish to have that name printed on the name tag in lieu of your first name, please e-mail this information to Ms. Tinisha Battle tbattle@health.usf.edu no later than July 18, 2008. Changes to name tags will not be made once school begins on August 11, 2008. 07/18/2008

  Create Oasis Account STEP 1: In an effort to protect personal information, all USF students have been assigned a system generated unique identification number that is to be used for all official university business. To obtain your assigned USF ID# (called your "U" number), go to the USF OASIS system at http://usfweb2.usf.edu/oasis/. Click on "OASIS Students/Register". Then select "OASIS Login" from the top left corner. Follow the instructions to obtain your U#. You must know your U# prior to completing many items outlined below ASAP
  USF ID Card STEP 2: Apply for a USF Student ID Card at the USFCard Center, which is located in the Phyllis P. Marshall Center, Room 105 on the USF Campus. Normal hours of operation are: Monday and Thursday, 9:00 a.m.-5:00 p.m.; Tuesday and Wednesday, 9:00 a.m.-6:00 p.m.; Friday, 8:00 a.m.-5:00 p.m.; and the first Saturday of each month, 9:00 a.m.-1:00 p.m. For more information, please call 813-974-2357.

If you are a medical student, obtain both a "USF Student" ID Card, and a "Medical Student" ID Card (you'll need to bring your medical school acceptance letter).  Ask that both cards be able to be used with the electronic doors at the USF Health buildings.  The medical student ID is worn when you are in the clinics and hospitals.

  Create USF COM Email Account STEP 3: Get connected to the USF Health email system!  Create a new USF Health computer account and gain access to email and other network resources while on and off campus.  Do this after you have created your Oasis Account, obtained your U#, and your Student ID Card.  https://hsccf.health.usf.edu/support_desk/account_request_pg_2.cfm ASAP
  Create USF Blackboard Account STEP 4: You will need to create and verify your USF NID so that you can access the Blackboard system which allows you to access course materials, grades, etc. Do this after you have created your Oasis Account, obtained your U#, and your Student ID Card. https://una.acomp.usf.edu/ 08/11/2008
  Immunizations Students must provide documented evidence of immunity to the communicable diseases listed below by completing the online Immunization Certification form https://hsccf.hsc.usf.edu/immunizations/md/ and submitting the specified supportive documentation to the Office of Student Affairs. All documentation must have the student's name clearly identified on every page/sheet; particularly when submitting documents in a "book" format. A list of acceptable documentation can be found on the Immunization Certification site. You must have your USF ID NUMBER to access this Immunization Certification site.
  • Rubeola, Rubella, Mumps Vaccination(s) (Measels/Mumps/Reubella) (MMR)
  • Tuberculosis Test (TST/PPD)
  • Titer-Verified Varicella immunity (Chicken Pox) or Varicella Vaccination
  • Tdap: Tetanus, Diphtheria, Acellular Pertusis Vaccination
  • Hepatitis B Vaccination (Requires series of 3 injections over 6 months)
  • Influenza (Flu) Vaccination

DO NOT DELAY!  Some immunizations require that you wait a period of time before you can receive the next dose in a series, others require a waiting period before you can obtain a verification titer.  Get this done as soon as possible!

IMPORTANT NOTE!: make sure they give you the TDAP immunization which includes Tetnus, Diphtheria, AND Accellular Pertusis.  Many clinics will administer a Tetnus booster to you rather than a Tdap, so ensure that they are giving you the right immunization.  If you receive a Tetnus, you will need to wait 2 years before you can receive the Tdap immunization.

  Health Coverage Students must have health insurance in effect at all times during enrollment at USF COM. During orientation you will be required to sign a statement certifying you have health insurance coverage. Annual re-certification is required throughout your enrollment. Information on basic health insurance policies will be available at orientation for students who do not have current health insurance. 08/11/2008
  Disability Insurance Students must have disability insurance in effect at all times during enrollment at USF COM. The policy will be purchased at registration on August 11, 2008 for approximately $72 and is to be renewed annually. Students may opt out of this requirement by providing proof of disability coverage. 08/11/2008

  Student Parking Pass You will need to obtain a non-resident parking pass to park on campus.  You can request a parking account #, wait to receive the account #, and then purchase one online here: http://usfweb2.usf.edu/parking_services/permits.asp 08/11/2008
  Purchase Laptop You will need to purchase a laptop and color printer for use while in school.  Financial aid can increase your student budget to accommodate this purchase, please contact financial aid for more information.  Goto the Computers & Tech Section for More Information. 08/11/2008
