Send an email describing request to firstname.lastname@example.org or fill out the request form at the USF Health IS Self-Service Portal (health.usf.edu/is).
There is no charge to the user for this service.
Contact the IS Services Service Desk by submitting a ticket via the USF Health IS Self-Service Portal, by phone at 813-974-6288 option #2, via email at email@example.com (for security related issues), or get walk-in help at the walkup desk located on the 2nd floor of the Faculty Office Building (MDF).
