College of Public Health students are required to complete the following forms prior to making travel plans:

Medical and Evacuation Insurance for International Travel

The State of Florida requires that all students traveling internationally for USF related activity maintain adequate medical and evacuation insurance for illness and injury. Before departure, students must show proof of medical and evacuation insurance coverage that is valid outside the US. In this regard, USF Education Abroad has a contract with Cultural Insurance Services International (CISI). The cost is reasonable. Please select the link below for travel insurance forms. Read the forms carefully, complete and sign them. Make copies for yourself and for your College's International Office. Submit the original completed and signed forms to the USF Education Abroad Office in the Patel Center. Keep a copy for yourself and submit a copy to the office coordinating your travel. (Note: Public Health students: For yourself, and for the International Field Experience office, attach a copy of the completed and signed insurance forms to your Application, Agreement and Plan. Submit one copy to the International Field Experience office. Retain a copy for yourself.)

If you plan to travel before or after your academic program, it is important that you purchase additional medical and evacuation insurance to cover the time period(s) outside your academic program.
