Faculty that are leaving USF Health are invited to participate in the Exit Interview process coordinated by the Office of Faculty Affairs.  Feedback from the short questionnaire and/or personal interview is given to the Dean for appropriate use. If you would prefer to have a personal interview,  contact  Office of Faculty Affairs, 813-974-1676, to make an appointment.
